There is still availability to sign up for our COVID-19 vaccination clinic at Middleburgh Jr./Sr. High School Gymnasium. Anyone 16 years and over can get vaccinated from 3:00 until 6:00 on Thursday, May 13. Please arrive for your appointment through the gym lobby entrance. You can park in the back lot.
Johnson & Johnson is a one dose vaccine that will be given to people 18 years and older.

The Pfizer BioNTech vaccine, FDA approved for people ages 16 and over, is reserved for our student population. Pfizer is a two-part vaccine. Students receiving the vaccine on May 13, will be required to receive the second dose at the same location and time on June 3. If you are not able to return on June 3, please do not register to receive the vaccine at this time.

Parental accompaniment is preferred for all students. All recipients who are under the age of 18 must have this consent form signed by a parent/guardian.
In the event students are not accompanied by a parent/guardian, the person who signed the consent form must be available by phone to confirm eligibility. A clinic staff member will call prior to the vaccine administration.
